  Some more details.
  I downloaded latest.zip and unzipped to a local drive. Viewed testDhtmlHistory.html in Firefox and it worked as expected. In IE the same thing does not create any history so the back button remains disabled. The simple demo from page 4 of the article works fine in the same IE instance.

  Just tried serving the files from a local Tomcat instance and it worked just fine. The problem only occurs when the URL is file:///...

  So what is known about the IIS issues? I have the exact same experience (simple example from page 4 works from O'Reilly site in IE; also works from my local site in Firefox but not IE -- the history is not registered). I am using IIS and don't have an option to change that. Any help would be great!
