The survey is hosted by Zoomerang, and I just tested it again. It isn't broken. It does have some mandatory flags that I don't like, but we'll have to live with them this time around. Please try to answer all of the questions. Many of them have an "other" option that you can write "none" in if the question doesn't apply.
